S\r‘i rS" , �`f WHEREAS , there exists an agreement between the City t'k,,\JL Qof Cape Canaveral, Florida and Communicable, a division of Al..,,,,,,,p7P- Southland Communications , Inc . , dated September 16, 1964, and a fifteen (15) year continuance of the franchise commencing rC, eptember 15 , 1979, which provides, in Section X thereof, for a reconsideration for the schedule of charges by the City Council; and ''M 12 WHEREAS , Communicable, Inc. has submitted a proposed rate schedule which the City Council has reviewed and approved; 'rNOW, THEREFORE, BE IT RESOLVED by the City Council 0 of the City of Cape Canaveral, Florida, as follows : SECTION 1. The rate schedule attached hereto and made part hereof, marked "Exhibit A" , is hereby established as the schedule of charges and installation fees under that certain agreement between the City of Cape Canaveral, Florida and Communicable, Inc . , which schedule of rates shall become effective within the City of Cape Canaveral on SECTION 2 . This Resolution shall become effective immediately upon its adoption. ADOPTED BY the City Council of the City of Cape Canaveral, Florida, this day of , 1983 . Mayor Attest: City Clerk Approved as to Form: City Attorney EXHIBIT A PROPOSED RATE SCHEDULE CONNECTION CHARGES: ALL CATEGORIES CHARGE FRANCHISE FEE Includes transfers, reconnects, move outlets 7 delinquent disconnects $35.00 Maximum SINGLE FAMILY DWELLING Initial outlet first month 8. 95 3% Each additional outlet 3. 00 3% Home Box Office and all other pay T.V. 3% COMMERCIAL Shops, banks, business offices, stores etc. First outlet per building 15. 00 3% Two or more outlets 7. 00 3% Home Box Office and all other pay T.V. 3% CONDOMINIUMS Building is pre-wired with internal master system and service is provided under a central billing plan regardless of number of TV outlets in an individual apartment. Charge for cable service will be included in monthly maintenance charge and paid monthly by the condominium association. Charge for cable service will be based upon total number of outlets in a building whether occupied or not and paid monthly by apartment manager/owner. Page 1 of 3 attached to Resolution No. 83-30 PROPOSED RATE SCHEDULE PAGE 2 PREWIRED HOTELS , MOTELS WITH MASTER INTERNAL SYSTEMS The following rates are based on connections to a service- able internal master antenna system serving entire establishment or complex under same management via central billing plan. Service charges are based upon total number of units per estab- lishment whether occupied or not. First outlet per account $8. 95 3% Each additional outlet under 300 units 2 . 00 3% Each additional outlet over 300 units 1 .50 3% Home Box Office and all other pay T.V. 3% GENERAL OPERATING POLICY The installation charge, first and last months service, transfer reconnect or additional outlet charges must be paid to the office before the work order is written. After the above charges have been received by Communicable, the work order wil be written; an account number assigned, and turned over to the installation department. Monthly Service Charges are payable on the first day of each month in advance. Assuming the Service Charge has been paid for the current month and the Service Charge for the following month is not paid by the 20th of the month, a bill and notice of a 1. 00 late charge will be sent. If the account is not brought up-to-date by the first of the following month, the deposit will be used to off-set that months Service Charge and service will be discon- nected on the first of the next month . EXAMPLE : You are paid thru January but not February. On February 20th, late notice is sent. Service will be disconnected on March 1st if no payment is received. A $5 .00 collection charge will be added to your account if the work order for a delinquent disconnect has been issued and a technician has to travel to your residence for collection. A $15 .00 reconnect charge will be assessed against all accounts disconnected for non-payment. A $10 .00 charge will be assessed for each returned check. Transfer of account from one subscriber to another is prohibited. All accounts must be terminated and reopened under the name of the new tenant or occupant with a new account number. If a service man is not required for the transfer the/cis a $10 . 00 transfer charge. If a service man is required, appropriate installation rates apply. Transient subscribers are urged to write or call for a disconnect when moving from a cabled residence. Account holders are liable for all service by the Company prior to the subscribers final notice to terminate serivce . Do not call answering service for disconnects as they are retained to process only trouble calls . Page 2 of 3 attached to Resolution No. 83-30 PROPOSED RATE SCHEDULE PAGE 3 The Cable system will respond to trouble calls only when the subscriber is on an individual basis . When trouble occurs in a bulk rate building, the Cable system will respond to trouble calls only when the manager/owner of the building calls. Then the Cable system will check out the signal strength of all channels going into the building system. The Cable system is not responsible for trouble occuring in the interior of the building, such as faulty wiring, wall plates, bad TV sets, etc. In the event it is necessary to install more than 150 feet of cable from lot line to TV outlet, an additional charge will be made at the rate of 75 per foot of cable plus $18 . 00 per man hour.
