All FDOT property shall be restored to its original condition. Any damage to FDOT property as a result of this work shall be repaired and restored in a manner acceptable to the FDOT at the sole expense of the permittee. In the event the permittee fails to meet any of the requirements of this permit by the FDOT, the permitted activity must cease until brought into compliance. If compliance can not be met, then the permit will be rendered void and said work shall be removed from the right of way at no cost to the FDOT. 2. Work shall commence within days of permit approval. S 9 0 /16 1. Notify the local FDOT Permit Office a minimum of 48 hours in advance of starting proposed work and a maximum of 48 hours after the completion of work [(321) 690- 3241 ]. Notification consists of calling and talking to an inspector. Calling and leaving a message on voicemail does not constitute notification. Office hours are 7:30 am to 5:00 pm. 2. At the time of notification, the permittee or his representative shall submit MOT job site ti supervisor and phone numbers of contact in case of an emergency, provide the permit number, permittee name, section number and roadway number. 3. It is the permittee's responsibility to comply with all Florida D. E. R. and the St. John's Water Management District and obtain any permits required from other governmental agencies. 4. A copy of the approved permit (including special provisions and plans) shall be on the site during all construction in FDOT right -of -way. Failure to have a copy of the permit in site at the time of inspection will result in a notice to stop work until such time as the permit or acceptable copy has been obtained. 5. When and where utility installation or associated activities interfere with pedestrian facilities, the permittee is responsible to ensure that an alternate route is provided that meets ADA requirements. Signage is to be in accordance with FDOT Design Standard Indices 600 series. 6. All construction and/or maintenance on the FDOT right -of -way shall conform to the Federal Manual of Uniform Traffic Control Devices, the FDOT Design Standards, the FDOT Standard Specifications for Road and Bridge Construction and the Utility Accommodation Manual. 7. The permittee is responsible for ensuring that each person supervising the selection and placement and maintenance of traffic control devices in the Utility Work Zone shall be certified by attending a FDOT approved MOT training class. A copy of these certifications is to be submitted to FDOT prior to starting work. 8. Lane closure restrictions may apply for the section of roadway in which the utility work is proposed. Contact the local FDOT Maintenance Office at (321) 690 -3241 for the current lane closure restrictions. 9. The permittee must comply with Section 335.15, F.S., requiring notification of the [ temporary closing of a FDOT roadway. Whenever any road on the State Highway System is repaired, reconstructed, or otherwise altered in a manner which necessitates the closin g of one or more traveled lanes of the roadway for a period of time exceeding two (2) hours, the party performing such work shall give notice to the local law enforcement agency, within whose jurisdiction such roadway is located, prior to commencing work on the project. 10. All portions of the FDOT right -of -way disturbed during construction shall be sodded with common Bermuda sod. 11. No Jack and Bore and/or Directional Bore shall be started after 9:00 am during the work day and no crossing will be started on a Friday, Saturday or Sunday. If a Jack and Bore is part of this permit: Permittee is required to submit a soils investigation report and a dewatering plan (if necessary). These items must be submitted at least two weeks prior to g P (� Y)• P starting the crossing operations. If a Directional Bore is part of this permit: A detailed report shall be submitted to the Department giving a written description, identifying the sequence of procedures and methods of the crossing. This report will identify the type of bore machine to be used and the amount of operator's experience. This report must be received and reviewed by the Department a minimum of two weeks prior to the start of the crossing. All permits that include a Jack and Bore and/or a Directional Bore will require a pre - construction meeting at the local FDOT permit office at least two (2) weeks prior to start of construction [(321)690- 3241]. 12. A vacuum truck will be required on all directional bores. Drilling fluid will not be permitted to be discharged into FDOT right -of -way. 13. All Jack and Bore and/or Directional Bore Pits will need to meet the requirements of the FDOT Design Standard Indices 600 series with regards to clear zone and drop off conditions. 14. Maintain erosion and sediment control devices per FDOT Design Standard Indices 100 series and N.P.D.E.S. permit (if required). All devices shall be installed before work begins in FDOT right -of -way. 15. Shoring will be required to conform with the "Florida Safe Trench Act" where necessary to protect the existing pavement, structures, foundations, etc. 16. Submit to the local Maintenance office, a copy of density reports, proctors, etc. for all driveway, roadway, sidewalk open cuts approved by the plans. 17. A pre - construction conference may be required for some projects not including a major Jack and Bore and /or Directional Bore. FDOT will inform the applicant/contractor at the time they call for notification of construction commencement. 18. As -built plans are required for all projects deviating from the approved plans. 19. The two selected stormwater inlets are as follows: • Chevron Gasoline Station — 8090 Astronaut Boulevard — southeastern corner of the intersection of SR AlA and Longpoint Road. • Wachovia Bank — 7801 North Atlantic Boulevard — southwestern corner of the intersection of SR Al A and International Drive. Photographs and aerial photographs showing the locations of the stormwater inlets are 1 attached. i 2). The stormwater filtration systems will be installed by the City's contractor, EcoSense International, Inc. (EcoSense) of Merritt Island, Florida. Installation time is estimated at two -three hours per inlet (total of four -six hours). Shop drawings of the stormwater ' filtration systems as provided by EcoSense are attached. 3). The City's Street Department no longer has any employees certified in maintenance of traffic operations. Therefore, certified personnel from one of the City's contractors, Atlantic Development, will be used for this project. Florida Department of Transportation x (FDOT) Index #612 will be used for maintenance of traffic — a copy is attached. k 4). The stormwater filtration systems will be added to the City's existing schedule for the 1 maintenance of nutrient- separating baffle boxes. Currently, the City uses an inmate crew R eceived ri A I s UG 2 3 2010 4-3r Operations 1 4 / to clean floatable debris from the boxes after each rainfall event. Therefore, the debris baskets associated with the filtration systems will be emptied after each rainfall event (or 3 monthly if no rainfall has occurred). 5). The City's Stormwater Administrator will be responsible for removing the stormwater 1 filtration systems in case of tropical storm/hurricane activity. The stormwater filtration systems will be removed 48 hours prior to landfall of any tropical storm or hurricane in the vicinity of the City.
