Dear Mayor Randels:
On behalf of the Florida Space Coast Council (FSCC) of the National Management Association (NMA), I
respectfully request that you proclaim the week of June 4th through June 9th, 2007 as Management Week in Cape
Canaveral.
During that week, the Association will sponsor its annual Management Week in America highlighting its objectives
to develop and recognize management as a profession through education and fellowship. Since its inception in 1978
by the NMA, Management Week in America has grown in recognition and activities each succeeding year.
The NMA is the largest not for profit organization of its type, with almost 21,000 members, that supports the
American Enterprise System and seeks to bring recognition of those leaders who advance the cause of quality in
American industry. John Quincy Adams, the 6h President of the United States, said, `If your actions inspire others
to dream more, learn more, do more and become more, you arc a leader." The Association is celebrating its eighty-
second anniversary this year. It was originally started as a Dayton Foremen's Club by Founder Charles Kettering and
grew into the international organization that we know today.
There are eight chapters in Florida, six of which are located in Brevard County and comprise the FSCC. The
organizations represented in the FSCC are: Boeing Space Coast Leadership Association, Central Florida Leadership
Association, Joint Base Operations Support Contract Leadership Association (JBOSC), Lockheed Martin Leadership
Association, Spacelift Range Systems Contract Chapter (SLRSC), and United Space Alliance Leadership
Association. They are located at Kennedy Space Center, Cape Canaveral Air Force Station, Patrick Air Force Base,
and the general Brevard County area.
The issuance of this Proclamation is appreciated and will be gratefully acknowledged at the FSCC Awards Banquet

INSTRUCTIONS FOR COMPLETING THE "REQUEST FOR STATE AND FEDERAL FUNDING" FORM
GENERAL: The "Request for State and Federal Funding Form" should be used by any local government or
transportation agency to nominate an improvement project that will require State and/or Federal funds to implement.
The form provides information on transportation improvement needs so that the MPO and its committees can
realistically and fairly evaluate one project relative to others of the same general type. The form should not be used for
projects relying entirely on local revenue, for transportation enhancement projects, traffic operations projects or for
projects using funds allocated by formula directly to agencies such as SCAT, airports, the port or the spaceport. A
separate form must be completed for each candidate project. Please include a response to each item even if the
response is "N/A." For convenience, the form may be filled out by hand, electronically (by downloading the MS Word
form from the MPO web site), or on separate sheets using a word processor, as long all questions are answered for
each project.
LINE 1: Enter the name of the local government or agency submitting the request and the year of the request
LINE 2: Check the appropriate category of the project. Descriptions of the categories are included in this package.
In order to be considered for SIS or RS funding a project must be located on a facility designated on the enclosed
map. Please note that Federal funds can only be programmed for projects included in the MPO 2025 Cost
Feasible (CF) Long Range Plan. The map is included on the MPO website (www.brevardmpo.com).
LINE 3: Indicate the relative priority of the project to ensure consideration of the most pressing local needs.
LINE 4: Check the appropriate description of the project request. "New" projects are those that were not submitted for
the MPO's consideration the previous year. "Revised" projects are those that have been previously submitted, but in a
different funding category, or with a different description, such as a change in project limits, or requested
implementation phase. "Continued support" indicates resubmission of a project with no changes. (Lines 7 —11 may be
left blank if no changes have occurred since last year.) "Support for another jurisdiction" indicates your support for a
project on a facility maintained by another jurisdiction. (Lines 7 — 13 may be left blank for projects in another
jurisdiction.)
LINE 5: Specify the road name and project limits or the intersecting street(s), as appropriate. Maps are not required but
can be submitted if needed to better define the project limits. Federal funds can only be spent on collector and arterial
roads (i.e., the federal aid highway system). State funds are limited to projects on the state highway system. Federally
funded projects not on the state highway system usually require 12.5% local match.
LINE 6: Describe the action requested for implementation with State or Federal funds such as "conduct planning study",
"design improvements", or "acquire right-of-way". Please note that prior to funding consideration by FDOT, projects
may be required to undergo Efficient Transportation Decision Making (ETDM) screening to determine, in part, if the
requested action is appropriate and feasible. For more information about the ETDM process visit

LINE 7: Provide a brief statement about the nature, extent, duration and consequences of the deficiency the project is
intended to address. An additional page may be attached to explain a complicated problem.
LINE 8: If a prior study was conducted, please reference the type of study (FDOT corridor study, city warrant study,
etc.), the completion date and the relevant study recommendation (s) that support the requested action on line 6. A
copy of the study should not be submitted, but should be made available, upon request. If additional project phases
(design, right-of-way) have been conducted, please indicate here.
LINE 9: Summarize any known public and/or private sector actions anticipated in the upcoming five years that may
substantially affect the nature or timing of the requested action. The intent is to properly define the scope of the
project, identify potential constraints and/or opportunities for public/private partnerships, right -of -way donations, etc.
INSTRUCTIONS FOR COMPLETING THE "REQUEST FOR STATE AND FEDERAL FUNDING" FORM
LINE 10: Summarize any public comment or actions by elected bodies regarding the requested project. The intent is
to gauge potential public support for or opposition to a project. Lack of public comment on a requested project will not
"count against" the project.
LINE 11: Summarize Comprehensive Plan or Master Plan support for the requested project. Projects without Plan
support will not be considered for funding by FDOT. Attach maps, tables and/or policy language, or note the specific
web address where these can be obtained for verification by FDOT prior to allocation of funding.
LINE 12: To assist MPO staff in preparing documents for the MPO's consideration, please provide a staff contact
name or department with a phone number and/or e-mail address in case there are any questions about this project.
LINE 13: Summarize any other information that will assist in the understanding of the requested project.
BREVARD MPO PROJECT PRIORITIES FUNDING CATEGORIES
Strategic Intermodal System (SIS)
The SIS includes transportation corridors and hubs that are essential to the economic vitality of the State of
Florida (as identified by FDOT and adopted by the Legislature in April 2004) and connectors designated by
FDOT in June 2004. In Brevard, projects located on Interstate 95, SR 528, and portions of US 192, Airport
Blvd., NASA Blvd., SR 401, and SR 405 (see enclosed map) are eligible for this funding. Projects at Port
Canaveral, the Spaceport, Melbourne International Airport and along the FEC railroad may also be eligible.
Although the MPO will recommend priorities for funding using SIS funds, FDOT is under no obligation to concur
with these recommendations. For further SIS information visit: http://www.dot.state.fl.us/planning/sis.
Regionally Significant Transportation Facilities (RS)
Based on FDOT guidelines and local knowledge of the transportation system, the MPO developed and adopted
a map of RS Facilities in Brevard. Designation was based on functional classification, hurricane evacuation and
connections to adjacent counties or regional facilities. A matrix indicating the rationale behind the designations
is included in the 2004 Project Priorities Status Report, which is on the MPO web site (www.brevardmpo.com).
Only projects located on the corridors indicated on the map are eligible for funding under this category.
Other Arterials and Collectors (STP)
Other projects not included on the SIS or RS systems are grouped for potential funding under the Federal
Surface Transportation Program (STP). STP funds are designated for expenditure based on urban area
population. "-A" (XA, SA, etc.) funds can be spent in any area. "-U" (XU, SU, etc.) funds can be spent in any
urban area with a population greater than 200,000. "-L" (XL, SL, etc.) funds can be spent in urban areas
with a population of less than 200,000. Programming control of XU funds lies solely with the MPO.
Programming of other STP funds is at the discretion of FDOT.
The Brevard MPO planning area includes the Palm Bay - Melbourne Urbanized Area, which includes the
area roughly from Malabar to Port St. John, Merritt Island and the Beaches, with a population greater than
200,000 and the Titusville Urbanized Area, which has a population less than 200,000. Additionally, a portion
of the Vero Beach - Sebastian Urbanized Area, with a population less than 200,000, lies within the Brevard
MPO area. Due to the multiple urban areas, the MPO has elected to prioritize projects in two subcategories
based on the population of the urban areas.
Transportation Enhancements
Projects such as bike paths, trails and landscaping that meet federal criteria are eligible for Transportation
Enhancement funds. Projects in this category undergo a separate application and ranking process by the
Bicycle, Pedestrian and Trails Advisory Committee (BPTAC) and may not be accepted every year.
Traffic Operations
Major operational improvements that provide moderate congestion relief or address safety concerns may be
considered for funding using funds set aside each year by the MPO for traffic operations. Projects in this
category undergo a separate evaluation and selection process by the Traffic Operations Committee and
may not be accepted every year.
