INDEPENDENT CONTRACTOR AGREEMENT
THIS ADDENDUM TO INDEPENDENT CONTRACTOR AGREEMENT ("Addendum") is entered on
this I day of Nat-ovtb-e{ , 2022 by and between the CITY OF CAPE CANAVERAL, a Florida municipal
corporation ("City"), whose address is 100 Polk Avenue, Cape Canaveral, Florida, 32920 ("City"), and
Hailey Haft, an individual, ("Contractor").
RECITALS:
WHEREAS, the City and the Contractor have entered into that certain Contract for Services dated
January '1 2022 ("Contract");and
WHEREAS, said Contractor desires to offer Zumba/Group Fitness ("Services") for the public using the
City's recreational facilities; and
WHEREAS, pursuant to the authority set forth in Article 5.4 of the Agreement,the City and Contractor
mutually desire to make minor modifications to the Agreement as set forth herein; and
IN CONSIDERATION of the mutual covenants and provisions hereof, and other good, diverse and
valuable considerations, the receipt and sufficiency all of which is hereby acknowledged, the parties
desiring to be legally bound do hereby agree as follows:
1. Recitals. The foregoing recitals are hereby deemed true and correct and by this reference are
incorporated herein as a material part of this Addendum.
2. Modified Services. Exhibit A of the Agreement is hereby amended to also include the provision of
Zumba/Group Fitness through December 31, 2023
3. Other Terms and Conditions. Any terms and conditions of the Agreement not expressly modified
by this Addendum shall remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto caused this Addendum to be executed as of the date first
written above.
