I made a few modifications to the spreadsheet (attached). I added in
the PWS ID for the City of Cocoa Water, I modified the Facility Name to
match what DEP has on file, and I concatenated the Street Number and
Street Name (to make all one cell) for the Physical Address.
Do you have the Street Suffixes for the Addresses? Also, do you have
any Parcel ID information for the Addresses? This will be most helpful
in matching all the data up for import into the GIS system.

Hello Levi,
Attached is the information we have available for our City of Cape
Canaveral.
The DOH template spreadsheet has been populated with the data we have.
Two more columns were added to facilitate the “Physical Street Address”
data requirements. You will have to merge this data before you upload
it to your main database.
Our City Waste Water Treatment Plant (WWTP) provides services to our
whole City except one property that is on a septic system.
Our whole City receives its drinking water from the City of Cocoa,
except for two properties that are on wells.
