For your information, please find attached photos for the non-permitted
overhead garage door at Puerto Del Rio.
This concerns damage which occurred today while the overhead door is
still in use.
This is the second occurrence since the door was replaced in January
2017.

Thank you for your response.
I must have misunderstood Officer Palmer a few days ago when he
indicated that you were handling the matter.
I wanted to make sure that the video about the garage door
crushing the level placed on the floor did transmit
properly to your e-mail and that you were able to view the
video.
Otherwise I will be glad to send it again.
From your response I take it that the Association will be
applying for a permit concerning the overhead garage door.
Please let me know if I did not understand your comments
properly.

Dear Mike,
Brian mentioned that you are handling the matter of the
overhead door at 8934 Puerto Del Rio Drive yourself.
While waiting to hear back from you, I was thinking
about something a maintenance person at the condominium told me last
month, that is that there was an accident at this same building in
February where the garage door ( it is on a timer) closed on a truck
while standing in the garage door opening.
The overhead garage door did crush the hood of the car,
did not reverse and caused substantial damage.
You may not be aware that the association did not only
replace the overhead garage door and attached old C-channels
to the back of it, but they also bought and installed a
new LiftMaster Commercial Door Operator (for garages accessible to the
public, like an apartment building) at the same time.
A copy of the invoice is available.
The problem is that the installation of all these items
was not done by a qualified garage door company, but rather by the
maintenance personnel of the condominium association.
It is of course unreasonable to expect the general
maintenance personnel to be aware of all the code issues and
installation requirements.
I do not know if the new Commercial Door Operator
required a permit, but it is apparent that there are major problems with
the installation.
One is that the new Operator has to be installed a
minimum of 8 feet from the floor ( the concrete ceiling is at 8 feet, so
this is an improper installation), the control station has to be away
from all moving parts of the door (it is right next to the overhead
door), there are no warning placards installed among other items.

The same requirements and more are in accordance with
the Florida Building Code which refers to Underwriters Laboratory
Standard UL 325 for detailed requirements.
Most important is the UL 325 requirement for a minimum
of two Entrapment-Protection devices.
At this time, there is only one Entrapment-Protection
device installed, the photo eye at 6" above the floor.
Imagine that there is a little girl on a tricycle under
the garage door while it closes. The chance of serious injury, if not
worse, is apparent.
To show the problem graphically, I did place a plastic
level, 2.5 inches tall, at the center of the garage door opening
and pressed the "close" button. As you can see, the
strong plastic broke easily under the pressure of the garage door and
the door did not reverse. It did not reverse after waiting about one
minute either. I tried this several times and the results were the same.
A person or extremities of a person or animal could be
pinned to the floor until help arrives. Injuries can be expected.
